The Department of Health and Human Services (the department) is committed to protecting your privacy. The department will collect and handle your survey response for the purposes of informing Victoria’s Allied Health NDIS Capability Framework project. Participation in this survey is voluntary.
Your de-identified responses may be disclosed to and considered by an expert panel, training provider and project steering committee. We may disclose de-identified information in your response in published documents including the capability framework, resources, training packages and reports. We may quote your response in these documents but we will not identify you.
This survey is anonymous, however, if you wish to be informed of updates on the consultation process and development of the framework you can provide your email address on the ‘stay up to date’ form. We will not share your email address with any third parties. Your email address will not be linked to your survey response.
If you do not provide us with the information requested in this survey, the department will not be able to consider your views and experiences into when developing the capability framework.
